Algorithm for Acute Cerebral Infarction

  1. Head CT to exclude intracranial hemorrhage, tumor or vascular malformation. May bypass head CT and proceed directly to MRI/MRA if feasible.
  2. MRI/MRA to evaluate extent of infarction and the degree of stenosis of cerebral vasculature.
  3. Carotid vascular ultrasound to complement MRA evaluation of extracranial carotid arteries.
  4. Carotid arteriogram in suspected cases of vascular dissection or vasculitis.
