Impression:

  • Mild-to-moderate increased LV volume.

  • Moderate-to-marked reduced uptake at rest in apex, the apical two thirds of the anterior wall, and anterior septum (arrows) with mild to moderate transient changes (lines) -- consistent with mixed transmural (apex and apical anterior wall) and nontransmural MI (11/25 segment) with moderate-to-severe ischemia in the residual and peri-infarct myocardium (7/25 segment) (LAD territory).

    Coronary Angiography:

  • LAD - 70-80% proximal stenosis followed by 50% mid-long stenosis
  • LCX - 2nd diagonal branch - 70% stenosis
  • RCA -luminal irregularities - 20% stenosis

    Comment:
    The functional significance of the above LAD findings is obvious but difficult to predict from pathoanatomy alone.
