Impression:

  • Moderate severe reduced uptake in the inferior lateral wall at rest, with mild stress-induced changes -- consistent with a mixed nontransmural / transmural (at the base) MI (arrows) (6/25 segments), mild ischemia in the residual myocardium (white lines), and peri-infarct ischemia (colored lines) (5/25 segments) (RCA territory).

    Coronary Angiography:

  • 70% RCA stenosis
  • All other coronary arteries widely patent

    Comment:
    There is a clearcut, severe MI in the RCA territory with 70% RCA stenosis. No clinical history of MI -- acute thrombosis on preexisting arteriosclerotic plaque with spontaneous thrombolysis is a likely explanation for this findings.
