Impression:

  • Fixed mild reduced uptake in the inferior wall from the apex to the base -- consistent with nontransmural MI (arrows) (RCA territory).

  • No ischemia detected at the good cardiac workload achieved.

    Comments:
    The fixed reduced uptake in the inferior wall stretches into the posterior septum, and to a lesser degree, in the inferolateral segments. In the sagittal long axis slices, the inferior wall appears inhomogeneous (bumpy). Myocardial injury is therefore more likely than attenuation.

    This patient had no known prior MI but had a single vessel CABG of 95% RCA stenosis 6 years prior to the present perfusion study.

    Recent coronary angiography showed a 50% stenosis of LCX artery, 50% stenosis of OM, and a 90% stenosis of native RCA, patent graft.

    LVEF = 50%.
