Impression:

  • Moderate severe decreased uptake in the lateral wall from the base to the apex (small lines) -- consistent with moderate-to-severe ischemia (6/25 segments; about 25% of the total myocardial volume) (LCX territory).

    Comments :

  • Notice the LV cavity "enlargement" between the rest and the stress study. The external dimension of the LV does not change. Therefore it is not a LV dilatation.
    Subendocardial ischemia in the lateral and inferolateral segments is the cause of the apparent increase in cavity size.
