Impression:

  • At rest, marked reduced uptake in septum and adjacent anterior wall (arrows) with mild reverse changes, and mild stress-induced ischemia in the apical-lateral segments (small lines)(LAD territory).
    The septum contains some viable myocardium, but no evidence of late fill-in to indicate hibernating myocardium.The degree of uptake in septum and adjacent anterior wall is in the order of 30% of max, thus improvement in function if revasculazed is quite unlikely.

  • No uptake in the apex and the inferior wall at rest and no late fill-in -- consistent with transmural MI (arrows) (7/25 segments) (RCA territory).

  • Only the basal two thirds of the lateral wall is well perfused and without evidence of injury or stress-induced ischemia (LCX territory).

    ECHO:

  • EF=20-30%
  • Extensive and severe wall motion abnormalities
