Impression:
Markedly increased LV volume (3+).
Mild to moderate increased RV uptake (green arrows).
No uptake in the apex, anterior septum, or the apical two thirds of the anterior wall -- consistent with huge anterior apical transmural MI (arrows) (11/25 segments, or 40-45% of total myocardial volume by quantification).
Moderate reduced uptake in the inferior wall with mild transient changes in the residual myocardium in the inferior wall and posterior septum -- consistent with nontransmural MI and mild ischemia in moderate volume of myocardium (small lines) (RCA territory).
Comments:
The degree of LV dilatation matches to the extent and severity of the rest perfusion abnormalities. The increased RV uptake indicates RV pressure overload, reflecting backward failure.
