Impression:

  • Moderate severe ischemia in the anterolateral wall (small lines) (4/25 segments) (OM1 or D1 territory).

    Coronary angiography:

  • LAD - 30% proximal, and 80% midle and distal stenosis
  • OM- Totally occluded proximally with left to left collateral filling
  • LCX - Mild diffuse disease
  • PDA - 90% stenosis
  • Mild anterior-apical hypokinesis

    Comments :
    Septum and the lateral wall are not ischemic. No perfusion abnormalities are detected in the inferior wall. Therefore, CAD in one of the small arteries is most likely.

    Insufficient collaterals to the anterolateral wall leads to ischemia (and symptoms) before the inferior wall or the anterior wall become ischemic. Therefore, only the most functionally significant stenosis was detected.
