Impression:

  • Markedly increased LV volume (3+).

  • Mild increased RV uptake (1+) (pink arrows with RV).

  • Mixed transmural / nontransmural MI in the apex and the apical two thirds of the anterior wall and septum (arrows) (7/25 segments), with mild ischemia in the residual myocardium in septum (small lines) (5/25 segments).

    Moderate extracardiac activity (liver) (arrows with "L") .

    Coronary angiography

  • LAD - Total occlusion of the proximal segment; 1st diagonal branch - totally occluded
  • OM - 95% stenosis
  • RCA (nondominant) - 95% proximal stenosis

    Comments:
    The marked increased LV volume matches the extent and severity of the rest perfusion abnormalities, consistent with ischemic dilated CMP.
