Impression:

  • Mild reduced uptake in the posterior septum (arrows) at rest -- consistent with but not diagnostic for nontransmural injury .

  • Moderate severe ischemia involving the inferior wall, posterior septum and apex (lines) (6/25 segments; 20% of the myocardium) (RCA territory).

  • The LV volume appears increased, but considering the patient's weight, the LV size is physiological.

  • There appears to be an "increase" in the LV cavity size from rest to stress. This is mostly an effect of ischemia in the septum and the inferior wall (assymetric "increase" in cavity size).

    Coronary Angiography:

  • Coronary angiography showed a right dominant system with normal coronary arteries.
  • Overall systolic function appears to be preserved. There were no focal wall abnormalities.

    Cardiac Cathetherization:

  • RV EDP = 44/ 20
  • PAP = 49/28/38
  • PCW = 32/31/28
