Note: ECHO data given below impression

Impression:

  • Markedly increased LV volume.
  • Moderately increased RV uptake.
  • Large mixed transmural / nontransmural MI in the apex and apical three fourths of the anterior wall and septum (arrows) (11/25 segments, 40% of total myoardium) (LAD territory).
  • Minimal peri-infarct ischemia in the anterior-lateral wall (small lines) (1/25 segments).

    ECHO

  • Moderate LV dilatation with moderate to severe reduction of systolic function. EF=about 25%.
  • Generalized global hypokinesis with multiple regional wall motion abnormalities including severe hypokinesis to akinesis of the anterior wall, septum, and apex.
  • Mild MR and TR (1-2+).

    Comments:
    Notice the extent and severity of the MI in the bull eye plots, only minimal stress induced peri-infarct ischemia detectable. Notice also that the extent and severity of the resting perfusion defects matches the LV volume.
