Impression:

  • Fixed mild reduced uptake in the inferior wall, posterior septum, and inferior lateral wall from apex to base (arrows) -- consistent with nontransmural MI (RCA territory).

  • Only minimal peri-infarct ischemia in the inferior lateral wall (white lines) was detected at the good cardiac workload achieved.

  • Prominent papillary muscle (posterior lateral) (green arrows).

    Comments:
    A potential pitfall may arise when the papillary muscle is the hottest portion of the myocardium. The whole study is then normalized to the hot papillary muscle, leading to a false impression of reduced perfusion in other portion of the myocardium (NOT the case with this patient).
