Cocaine Abuse
Case history/ Imaging data

SPECT myocardial perfusion studies were performed twice (shortly after admission, and after one week treatment and cocaine abstinence) using Tc-99m-Sestamibi. Images representing myocardial perfusion were obtained at rest and peak stress.
STUDY 1
(Shortly after admission)
Stress protocol
  • Treadmill / Bruce
  • Duration of stress
  • 12 min
  • Peak heart rate (85%)
  • 70 --> 118 (85%=150)
  • Systolic BP (basal --> peak)
  • 110 --> 120
  • Double product (peak rate x BP)
  • 14,160
  • Reason for termination
  • Chest pain
  • ECG findings
  • 2 mm ST depression

  • STUDY 2
    (Post 1 wk treatment and abstinence from cocaine)
    Stress protocol
  • Treadmill / Bruce
  • Duration of stress
  • 11 min 25 sec
  • Peak heart rate (85%)
  • 70 --> 138 (85%=150)
  • Systolic BP (basal --> peak)
  • 180 --> 130
  • Double product (peak rate x BP)
  • 23,400
  • Reason for termination
  • Fatigue. No chest pain
  • ECG findings
  • Upsloping ST changes
