Comment:
Mild reduced uptake in septum (7 to 11 o'clock) and no involvement of apex is the typical pattern of LBBB-induced hypoperfusion.
Increased intramural pressure caused by the abnormal septum contraction reduces the effective perfusion pressure leading to hypoperfusion.
A similar pattern can be seen in patients paced form RV (functional LBBB).
