Impression:

  • Markedly increased LV volume (3+).
  • No fixed or reversible perfusion abnormalities detected.
  • Mild increased RV uptake (arrows with "RV") in the rest and stress study.
  • The absence of perfusion abnormalities in the setting of markedly increased LV volume strongly suggests LV volume overload condition (CMP ?, valve disease ?).

    ECHO

  • LV dilatation with severe reduction in systolic function
  • EF=15-20%
  • Mild mitral regurgitation (1+)
  • Left atrial enlargement

    Comments:

  • Most likely explanation for the contractile dysfunction is hypertensive CMP, and a less likely theory would be primary CMP.
